Day Care Services for Adults is the provision of an organized program of services during the day in a community group setting for the purpose of supporting adults' personal independence, and promoting their social, physical, and emotional well-being. Services must include a variety of program activities designed to meet the individual needs and interests of the participants, a nutritious meal and snacks as appropriate to the program, and referral to and assistance in using appropriate community resources. Medical examinations are required for individual participants for admission to a program and periodically thereafter. Services must be provided in a home or center certified to meet state standards for such programs.

Congregate Nutrition
Congregate nutrition provides a hot meal at a social setting.
The meal meets 1/3 of the daily RDA requirement for nutrition
and is typically prepared by a food catering services. Congregate sites,
which include senior centers, churches and community centers, are available in all nine counties
and sometimes transportation is available.

Consumer Directed Care
Consumer directed services give individuals the ability to make
decisions about their home care, from who to hire to what
services are needed. Services could include housecleaning, meal
preparation, or a trip to the grocery store. The ultimate goal
of this program is to allow older adults to manage their
services in the manner which best suits them, and allows them to
stay in the community for as long as possible. Currently in Region F, only Cabarrus county
offers this service but the state of North Carolina is looking
to expand this across the state.

Home Delivered Meals
Home delivered meals provide a nutritious meal to vulnerable
elders at their home. Vulnerable elders are defined as those who
are not able to get out of their house, due to various physical
or mental limitations. Most services are provided to those age
60 and over, but some not meeting that requirement may qualify
under "special eligibility." To see if you are

Housing and Home Improvement
Several counties have selected to offer housing and home
improvement. This service provides minor home repairs and
modifications, such as ramps and grab bars, to eligible
individuals. Currently, Lincoln County and Cabarrus County are
the only two counties in the region that offer this service.

Information & Assistance
Most counties in the nine county region are funded for
information and assistance. This service provides comprehensive
information, referrals and assistance to consumers regarding

In home aide services are intended to assist individuals who have functional/ physical and/or mental impairments with essential daily activities in the areas of home management and personal care tasks. These services enable individuals to remain in their home settings for as long as possible. The services may also provide assistance and respite to families who are involved with care giving responsibilities.
One type of in home aide service is personal care. Personal care, which is more extensive and complex in nature, is provided by a certified nurse aide employed through a licensed home care agency. Services that are considered personal care in nature can include help with bathing, dressing, toileting, feeding, and ambulation.
The other type of in home aide services is home management. Home management services can include tasks such as cooking, cleaning, laundry, shopping, and other non-personal care activities to keep the individual safe in the home setting. Home management tasks are provided by agencies that are Certified by the Division of Aging and Adult Services.
Clients eligible for services must be 60
years of age or older, live at home, and have home management and/
or personal care needs. For more information on services available

Transportation services provide travel to and/or from community
resources such as medical care facilities, nutrition sites, or other
locations for the purpose of accessing needed services or carrying
out activities for daily living. Transportation Services are limited
to individuals 60 years of age and older who are in need of the
