Evidence-based health programs are interventions that are based on evidence that is generated by scientific studies and whose results have been published in industry and medical journals. They have been rigorously studied, tested and proven to provide positive health effects for participants.

Living Healthy: Chronic Disease Self Management Program

Living Healthy- Feel Better, Take Charge is the Chronic Disease Self Management Program from Stanford University. This six week program is for individuals living with a chronic condition. It emphasizes empowerment and personal control over one's health care and encourages confidence and motivation for participants, teaching them how to be active in their own healthcare.

Arthritis Foundation Exercise Program
An eight session program developed by the Arthritis Foundation that strives to reduce, manage and understand the many signs of arthritis. Participants are assisted in creating a personalized exercise plan compatible with their everyday needs.

Classes are comprised of both exercise sessions and an education component. Currently, all nine counties offer this program. For more information about class offerings, contact the agency in your county.

A Matter of Balance: Managing Concerns About Falls
Many, many older adults live in fear of falling which often results in hospitalization, long recuperation periods and loss of independence. This program emphasizes practical strategies, to reduce the fear of falling and increase activity levels among older adults. Participants learn to view falls and fear of falling as manageable. They set realistic goals to increase activities, learn to control their environment to reduce fall risk factors, and exercise to increase their strength and balance.

The eight week session is lead by a team of two certified coaches who assist the small group of 10- 15 seniors learn to make changes to reduce their chances of falling and be more in command of their daily lives.

Walk with Ease
Developed by the University of North Carolina, this program is helpful for older adults who find it difficult to maintain a regular exercise schedule. Participants are motivated and encouraged to develop and follow a walking program that incorporates their individual health needs.
